cannot get code or access account

After I enter my usename and password, I am told I need a code. But when I request the code, it does not send me one and I get an error page that says I have hit a snag. Then it kicks me out. I can no longer access my account no matter if I use Windows, my Mac or my iPhone and no matter which browser I use.
