Cannot purchase minutes

Get stuck at the order screen and cannot ever pay for it. Tried for 2 days now. No way to report it on the page without having to look up IMEI or phone number either. (I did not have the phone on me as it is my daughters. Yes I could look it up but there should be a simpler way to say IT DOES NOT WORK.)
