Website Error on Seller Log In Page

Everytime I try to log on via the Seller Account Log In page, I am prompted with an error message stating "There was a problem with your request. Missing email address. Please correct and try again." However, I am inputting both my email address and password (both are correct and complete) and still getting this error message. When trying to log on multiple times, eventually the error message disappears, but I am still brought back to the log in page without being logged on. After the error message disappears, it seems as though the 'Forgot Your Password?' and 'Help' links are misplaced and sit below the log in box instead. Please see screenshots attached.
